What is pediatric compounding?
Pediatric compounding is the preparation of a customized medication for a child by a licensed pharmacy, made to a prescriber's written specification. It changes an existing drug's concentration, flavor, or dosage form when no commercially available product suits the child's weight, age, or tolerances.

A tablet a young child cannot swallow

Sometimes the problem is a dye, sweetener or preservative rather than the active ingredient. Where the formulation allows, we can prepare it without the specific ingredient.
Food allergies and flavoring
Flavorings are food derived. Tell us about allergies before the preparation is made, since removing something afterward means starting again.

Very small doses

Straight answers about what we cannot do
We are a non sterile pharmacy. We do not prepare injectables or anything requiring sterile technique. We also will not guess at a formulation. If an active is a poor candidate for a particular flavor or vehicle, we will say so on the first call rather than send home something a child will refuse.

Safety information
Most medication problems in children are not caused by the drug. They are caused by how it was measured, stored, or reached. Please read this section before the first dose.
Measure with the oral syringe, never a kitchen spoon
Doses for children are written in millilitres, and household spoons vary enormously in size. Use the oral syringe or measuring device supplied with the prescription, read the dose exactly as written on the label, and ask the pharmacist to show you the mark if you are unsure. If a dose is missed, do not double the next one. Call your prescriber or the pharmacy instead. If you think a child has received too much, contact your prescriber immediately, and in the United States you can reach Poison Control at 1-800-222-1222.
Flavored medicine can look like a treat
Flavoring exists so a child will accept a dose, and it works. It also means the preparation may look and taste appealing to a child who finds it unsupervised. Store every medication up high, out of sight and out of reach, and closed. Never describe medicine as candy while giving it. Keep it in the original labeled container rather than decanting it into something else.
Shake it, store it as labeled, and stop on the date given
Suspensions separate between doses, so shake the bottle well each time unless the label says otherwise. Some compounded preparations require refrigeration and many carry shorter beyond use dates than manufactured products. Follow the storage line on the label and discard the preparation on the date given rather than keeping leftovers for a future illness.
Tell the prescriber and the pharmacy about allergies
Children sometimes react to a dye, flavor, sweetener or preservative rather than to the active ingredient. Tell us before the preparation is made, since removing an ingredient afterward means starting again. Mention any reaction to a previous medication, however minor it seemed, and tell us if the child has an allergy to foods that appear in flavoring.
One child, one prescription
A compounded preparation is made for a named child at a named weight. Never give it to a sibling, never reuse a preparation from a previous illness, and never adjust the amount because a child seems bigger or smaller than last time. Speak with the prescriber before changing anything.

Compounded preparations are not FDA approved
They have not been reviewed by the FDA for safety, effectiveness, or manufacturing quality. Many medications used in children do have FDA approved pediatric liquid formulations. Where an approved product suits the child, it is generally the appropriate first choice.
Prescription required, every time
Every preparation is made for an identified child from a prescription written by a licensed prescriber. We cannot begin from a parent request alone, and compounded medications are not available over the counter.
Non sterile scope
Oral liquids, capsules, troches, topicals and suppositories. No injectables, nothing requiring sterile technique.
Weight on the prescription
Pediatric prescriptions should carry the child's weight so the pharmacist can sanity check the concentration and volume against the dose.
USP 795 and USP 800
Our lab follows USP 795 for non sterile compounding and USP 800 for hazardous drug handling, with a negative pressure containment room.

Taste is not a nicety, it is whether the dose gets taken
A child who spits out a bitter suspension has received an unknown fraction of the dose, and the parent has no way to know how much. Matching the flavor to the child and to the active is part of the formulation, not decoration added afterward.

Do I need a prescription to have my child's medication compounded?
Yes. Every compounded preparation is made individually under a valid, patient specific prescription from a licensed prescriber. Your child's doctor can call (909) 378-7301 or fax the prescription to (909) 378-7071.
How should I measure my child's dose?
Use the oral syringe or measuring device supplied with the prescription, and read the dose exactly as written on the label in millilitres. Household spoons vary in size and should not be used. Ask the pharmacist to show you the correct mark if you are unsure.
