What Is Compounded Hormone Replacement Therapy?
Compounded HRT is a customized hormone preparation made by a pharmacy to a prescriber's written order. It lets a provider choose the exact hormone, strength, and form for one patient when a commercial product does not fit.
When a Provider Considers Compounded HRT
A prescriber may order a compounded hormone preparation when an FDA approved product does not offer the needed strength, combination, or form, or when a patient reacts to a dye, filler, or preservative. Common reasons include:
- A strength not made commercially
- A dye free or allergen conscious version
- An alternative form such as a capsule, troche, cream, gel, or vaginal suppository
- A combination a provider wants in a single preparation
Which Hormones Mixwell Can Compound
Mixwell prepares common bioidentical hormones by prescription, alone or in prescriber selected combinations:
- Estradiol and estriol (estriol is not FDA approved in the United States and is available only through compounding)
- Progesterone
- DHEA
- Testosterone (a Schedule III controlled substance)
- Combinations such as Biest (estriol with estradiol)
Who Prescribes Compounded HRT
Providers across women's health, men's health, and hormone care order compounded HRT for their patients. Common prescribers include:
- OB/GYN and women's health NPs and PAs
- Endocrinology
- Menopause and midlife clinics
- Functional, integrative, and anti aging medicine
- Men's health clinics (testosterone and related therapy)
- Primary care, internal medicine, and naturopathic doctors

Because hormones are handled as hazardous drugs, we compound under USP 795 for nonsterile compounding and USP 800 for hazardous drugs, using HD negative pressure containment. As a PCCA member pharmacy, we prepare each formula for one patient by prescription. Mixwell is a nonsterile pharmacy and does not make sterile products or injectables.
