What is men's health compounding?
Men's health compounding is the preparation of a customized medication by a licensed pharmacy, made to a prescriber's written specification. It adjusts an existing drug's strength, combination, or dosage form when no commercially available product fits the patient.

A household with young children
Commercial testosterone gels carry a risk of transfer through skin contact. Some prescribers move a patient to a different route rather than manage that risk at home.
The patient who will not use a gel
A troche dissolves in the mouth instead of sitting on the skin. Absorption varies with technique, so it suits patients who will follow the directions consistently.
Raising testosterone without suppressing sperm
Testosterone given from outside the body suppresses sperm production. Some prescribers consider agents that work through the body's own hormonal axis instead. Not FDA approved, and used off label.
Multiple strengths for one patient
When a prescriber expects to adjust over time, we can prepare more than one strength, or score a troche so a dose can be divided along a marked line.
A form other than a swallowed tablet
Oral sildenafil and tadalafil are FDA approved and generally the first choice. Compounding covers combinations and routes no manufacturer produces. Read the nitrate warning on this page first.
Above the over the counter strength
Topical minoxidil is approved at 2 and 5 percent over the counter. A prescriber can specify a higher strength, or a combination in one daily application.
Finasteride in a shared home
Finasteride can affect the development of a male fetus, and that is a handling precaution as well as a use precaution. Worth raising at the visit, not just on the label.

Straight answers about what we cannot do
We are a non sterile pharmacy. We do not prepare injectables, subcutaneous pellets, or anything requiring sterile technique. Injections are the most common route for testosterone therapy, so knowing this before you write saves your staff a call. If a request falls outside our scope we will say so on the first call rather than waste your time.
Read before prescribing or starting
Safety information
Changing the dosage form does not change the drug. Everything below applies to a compounded preparation exactly as it applies to a manufactured one.
Never combine PDE5 inhibitors with nitrates
Sildenafil, tadalafil and other phosphodiesterase type 5 inhibitors must not be taken with nitrate medications. The combination can cause a sudden, severe and potentially life threatening drop in blood pressure. Nitrates include nitroglycerin in any form, isosorbide mononitrate and dinitrate, and recreational substances known as poppers. Tell your prescriber every medication you take, and tell emergency responders you have taken a PDE5 inhibitor if you develop chest pain. Caution also applies with alpha blockers and some blood pressure medications. Seek immediate medical attention for an erection lasting more than four hours, or sudden loss of vision or hearing.
Testosterone side effects and status
Testosterone can raise red blood cell counts, and an elevated hematocrit is a common reason therapy is adjusted. Product labeling carries a warning that testosterone products increase blood pressure, and warnings about blood clots in veins. Other reported effects include acne, breast tenderness, fluid retention, worsening sleep apnea, mood changes, reduced testicular size and reduced sperm production. Testosterone is a Schedule III controlled substance and its labeling carries a warning regarding abuse and dependence. FDA has retained limitation of use language stating that safety and efficacy are not established for low testosterone attributed to aging alone. This list is not complete.
Finasteride handling precaution
Finasteride can affect the development of a male fetus. Women who are pregnant or who may become pregnant should not handle finasteride containing preparations. This is a handling precaution as well as a use precaution, so it matters in any shared household. Follow the directions on the prescription label.
Evaluation comes before a preparation
Erectile difficulty can be an early indicator of cardiovascular disease. Reduced desire can reflect thyroid or hormone changes, depression, sleep problems, chronic pain, or the side effects of medications already being taken. None of those are addressed by a compounded preparation, which is why evaluation by a prescriber is the part most likely to help.

Compounded preparations are not FDA approved
They have not been reviewed by the FDA for safety, effectiveness, or manufacturing quality. FDA approved products exist across men's health, including testosterone in several dosage forms, oral sildenafil and tadalafil, and topical minoxidil. Where an approved product fits the patient, it is generally the appropriate first choice.
Prescription required, every time
Every preparation is made for an identified patient from a prescription written by a licensed prescriber. We cannot start a preparation from a patient request alone, and compounded medications are not available over the counter.
Non sterile scope
Oral, topical, transdermal, sublingual and nasal preparations. No injectables, no pellets, nothing requiring sterile technique.
Controlled substances
Testosterone is Schedule III. Those prescriptions must be transmitted in a manner that meets controlled substance requirements.
USP 795 and USP 800
Our lab follows USP 795 for non sterile compounding and USP 800 for hazardous drug handling, with a negative pressure containment room.

Federal law requires that prescription drug promotion, including promotion of compounded drugs, be truthful, accurate and not misleading. Do I need a prescription for compounded men's health medications?
Yes. Every compounded preparation is made individually under a valid, patient specific prescription from a licensed prescriber. Compounded medications are not available over the counter and cannot be ordered directly from the pharmacy.
Does Mixwell compound testosterone injections or pellets?
No. Mixwell performs non sterile compounding only. We do not prepare injectables, subcutaneous pellets, or any sterile preparation. Those require a pharmacy licensed for sterile compounding, or an FDA approved product.
Are compounded men's health preparations FDA approved?
No. Compounded preparations are not FDA approved. Approved products exist across these categories, including testosterone in several dosage forms, oral sildenafil and tadalafil, and topical minoxidil. Where an approved product fits the patient, it is generally the appropriate first choice.
Can I take a PDE5 inhibitor if I take nitrates?

Is testosterone approved for low testosterone due to aging?
No. FDA has retained limitation of use language stating that safety and efficacy have not been established for low testosterone attributed to aging alone. Testosterone products are approved for men with hypogonadism caused by a specific medical condition, confirmed by laboratory testing.
Will testosterone therapy affect my fertility?
Testosterone given from outside the body suppresses the hormonal signals that drive sperm production and can substantially reduce sperm counts. This applies to all routes. Discuss fertility plans with your prescriber before starting.
How long does compounding take?

Can I use my HSA or FSA to pay for a compounded medication?
Yes, you should be able to charge compounded medications for people to your HSA or FSA account. Mixwell does not accept insurance. Call (909) 378-7301 for pricing.
How is a compounding pharmacy different from a regular pharmacy?
A retail pharmacy dispenses manufactured products as they come from the factory. A compounding pharmacy prepares a medication from individual ingredients to match a prescription, changing the strength, combination, or dosage form. Compounded preparations are not FDA approved.
