Clinical Overview

What makes dental compounding different

Dental compounding works against constant washout. Saliva dilutes and clears whatever is applied to the mouth, and swallowing and chewing add mechanical stress, so a preparation can leave the tissue quickly [1]. Mucoadhesive vehicles are designed to hold a preparation against the mucosa for longer [2].

Compounded mouthwashes are studied in this setting. A phase 3 trial in 275 patients receiving head and neck radiotherapy compared doxepin mouthwash, a diphenhydramine, lidocaine, and antacid mouthwash, and placebo for oral mucositis pain, and both active mouthwashes reduced pain more than placebo over the first four hours [3].

A rinse, a gel, and a troche behave differently in the mouth, so our pharmacist reviews the vehicle with the prescribing office before compounding. Prescriptions should name each active and strength, the base, the quantity, and the directions, including whether the patient swishes and spits or swishes and swallows. Common Questions

How does a prescriber send a compounded prescription to Mixwell?

Prescribers can call, fax, or send an electronic prescription. The order should name each active ingredient and its strength, the base or vehicle, the dosage form, the quantity, and the directions. Our pharmacist confirms anything unclear before compounding, and can talk through vehicle options for the oral cavity beforehand.

Can a compounded oral rinse or gel be flavored?

Yes, when the prescriber orders it. Flavoring is often the difference between a patient finishing a course and abandoning it, especially for children and for patients using a rinse several times a day. Sugar free options are available for patients with dry mouth or dental concerns.

Do compounded oral rinses and dental gels require a prescription?

Yes. Every compounded preparation is made for one patient with a valid prescription from a licensed prescriber, including dentists. Mixwell cannot prepare or dispense these without an order, and the prescriber decides the ingredients, the strengths, and how the preparation is used.
