How It Works

What is clindamycin?

Clindamycin is a prescription antibiotic that stops certain bacteria from making the proteins they need to grow. It is used for serious infections caused by bacteria that respond to it [1]. It does not work against viruses, such as colds or the flu.

What is the dosage form?

Clindamycin 10 mg/mL Oral Suspension is a liquid medicine you swallow, made by a compounding pharmacy to a prescriber's order. Each milliliter contains 10 mg of clindamycin. It is shaken before each dose and measured with an oral syringe or dosing cup.

What SuspendIt is designed to do

SuspendIt is the liquid base that holds the clindamycin. Its manufacturer describes it as follows:

  • Designed to keep the medicine evenly spread through the liquid, so each measured dose after shaking contains the intended amount
  • Thick when resting and thinner when shaken, which helps it pour and measure easily
  • Made without sugar, which matters for patients who need to limit sugar
  • Made without dyes, for patients who react to or want to avoid them
  • Able to be flavored to the prescriber's or patient's preference

When prescribers choose it

  • Prescribers who want a 10 mg/mL strength rather than the commercial 15 mg/mL liquid, for example for small doses measured by weight
  • Prescribers whose patients need a sugar free or dye free liquid
  • Prescribers whose patients cannot swallow capsules

Clinical Details

What does the evidence show?

FDA approved clindamycin is available as capsules and as Cleocin Pediatric granules that are mixed with water to make an oral solution of 75 mg per 5 mL, which is 15 mg per mL. Its labeling covers serious infections caused by susceptible bacteria, and it carries a boxed warning for Clostridioides difficile associated diarrhea [1].

Findings come from the cited studies and do not promise any individual outcome.

Why is it compounded, and how is it typically used?

Why it is compounded

An FDA approved clindamycin liquid exists at 15 mg/mL [1]. Federal law restricts compounding preparations that are essentially copies of approved products unless the prescriber documents a significant difference for the patient, such as a different strength, a sugar free or dye free base, or a different flavor [2].

Dosage forms and strengths

  • Clindamycin 10 mg/mL, or a strength the prescriber sets
  • SuspendIt suspending base, with flavoring on request
  • Dye free and sugar free
  • Dispensed with directions to shake well before each dose

Dosing and duration are the prescriber's decision.

Safety information

FDA labeling for clindamycin carries a boxed warning: Clostridioides difficile associated diarrhea has been reported with nearly all antibiotics, including clindamycin, and can range from mild diarrhea to fatal colitis. If significant diarrhea occurs, contact the prescriber, and the antibiotic may need to be stopped [1].

The labeling also advises caution in people with a history of stomach or bowel disease, especially colitis, and in people with allergies. Clindamycin can lead to yeast overgrowth, and antibiotics used without a bacterial infection add to drug resistance [1]. Take every dose as prescribed. Patient selection, dosing, and duration are the prescriber's responsibility.
